Scheduling Instructions:
- Print and COMPLETE the New Patient Form above prior to your appointment time. (These need to be complete before your appointment time, or you may receive a shorter session as we need to stay on schedule for the following clients.)
- If you have Pre-Purchased by voucher or credit card please continue. (If NOT pre-purchased, please call 801-621-2251).
- Please complete steps 1-4 for EACH appointment.
- Please use First and Last name when scheduling.
- An email confirmation notice will be sent immediately. If not, please check your spam folder.
- If you have multiple appointments, please schedule ALL of your appointments. At the soonest 48 hours apart, at the longest one week apart.
- Please schedule ALL purchased appointments. If you wait, availability is based on the schedule.
- All available appointments are listed. (If an appointment is canceled, the appointment will show available.)
- PLEASE be sure to WRITE down your appointment time AND arrive for your appointment on the correct day and on time. ALL SERVICES are provided at our location in Sandy UT.
- As a consideration to all of our clients (and our equipment), please do NOT bring children with you to your appointment. Please note that if you arrive to your appointment with children, your appointment will be canceled.
We adhere strictly to these Policies:
- 48 hour Appointment Cancellation Notice is REQUIRED or service will be billed, or if applicable, voucher will be voided. Please note: If you must change your appointment time(s) be sure to cancel any previously booked appointments or you may be billed for them.
- UNSCHEDULED ARRIVAL: If you arrive for treatment when you do NOT have an appointment scheduled, you will be asked to return at your scheduled time or you will be billed an additional fee for same day LipoLASER scheduling. If you are unsure of your appointment times please email or call to confirm your scheduled times.
- EXPIRED VOUCHER: If your voucher has EXPIRED, please do NOT schedule an appointment here. If your voucher has expired please call 801-621-2251 to discuss with staff member.
- ALMOST Expired Voucher: If your voucher is pending expiration, please schedule all of your appointments. We do not need to see you before it expires, but we do need to have you on the schedule to complete your treatments within 3 weeks of the expiration date.
- Valid Emergency: In the case that there is a valid emergency and an appointment can NOT be kept, an appointment can be rescheduled by paying a $75 rescheduling fee. The availability of this option will be determined by the doctor.
- To Cancel an Appointment: In the event that you need to cancel an appointment that you scheduled using our Online Scheduler, please refer the the email confirmation that was sent. At the bottom of the email, there is a link where you may cancel. Then choose new appointment time(s). If you have any questions, simply give us a call, 801-621-2251. 48 hour notice is required or the treatment will be voided.
Appointment Options:
- OPTION A. FIRST LipoLASER Clients: If it is the FIRST time we will be seeing you for LipoLASER, Please select Option A. AFTER you have scheduled your FIRST LipoLASER Appointment, select either Option A or B for your additional appointments.
- OPTION B. ADDITIONAL LipoLASER Appointments. Not for FIRST LipoLASER appointments, ONLY Additional.
- OPTION C: Body Wraps, Ultrasound Cavitation and Radio Frequency Treatments please use Option C.
- Electro Body Sculpting Clients: Treatment will be completed at one of your LipoLaser appointments. No additional appointment is necessary
